atty liver means fat has started collecting inside your liver cells, where it shouldn't be. A healthy liver contains almost no fat. When more than 5% of the liver is made up of fat, it is called fatty liver.
The early stage is usually silent — no pain, no obvious symptoms. Over time, if nothing changes, the fat can lead to inflammation and eventually scarring. The good news: in its early stage, it's highly reversible.
NAFLD stands for Non-Alcoholic Fatty Liver Disease — fatty liver in people who don't drink much or any alcohol. It's the kind most Indians have today, driven by sugar, refined carbs and belly fat rather than drinking.
"Fatty liver" is the everyday term. NAFLD is the medical label for the non-alcohol type specifically. The alcoholic type is a separate condition with the same appearance on scans.
Grades describe how much fat the ultrasound sees in your liver:
Progression isn't automatic — it depends on your habits, weight and other health conditions. Many people stay at Grade 1 for years. Others move through grades in months. The difference is lifestyle.
Yes — in its early stages, fatty liver is one of the most reversible conditions in medicine. Most people reverse it by losing 7–10% of their body weight through diet and exercise.
Scarring (called fibrosis) is harder to reverse but still partly reversible when caught early. The one stage that doesn't reverse is full cirrhosis — which is exactly why acting early matters.
Not automatically — but fatty liver, insulin resistance and heart disease share the same root causes (belly fat, high sugar, lack of movement, poor sleep). So they often travel together.
People with fatty liver have a higher risk of developing type 2 diabetes and heart disease over time. Reversing the fatty liver usually improves both risks at the same time, because the same lifestyle changes fix all three.
A good baseline for suspected fatty liver:
If results are abnormal or unclear, a FibroScan (a non-invasive scan that measures liver stiffness) can show whether scarring has started.
These are enzymes made by the liver. When liver cells are stressed or damaged, they leak out into the blood. So high levels can mean the liver is under load.
Possibly. Around 25 to 30 out of every 100 people with confirmed fatty liver have completely normal blood tests. The liver can be storing fat quietly without leaking any enzymes into the blood.
If you have belly fat, high fasting sugar, or persistent symptoms like right-side heaviness or afternoon fatigue, it's reasonable to ask your doctor about an ultrasound — regardless of what the LFT shows.
Broad guidance (your doctor will tailor it):
Most people don't. FibroScan — a painless 10-minute scan — is only needed when your doctor suspects scarring (fibrosis) might have started. It's usually the next step after an abnormal ultrasound or persistently raised enzymes.
Liver biopsy is rarely needed anymore. It's reserved for complex cases where the diagnosis can't be confirmed by scans and blood tests. For routine fatty liver, you almost never need it.
The Mediterranean-style pattern has the strongest evidence for reversing fatty liver. Translated for an Indian kitchen, that looks like:
You don't have to be perfect. Aim for 80/20 — 80% good, 20% flexibility.
Large population studies consistently find that people who drink 2–3 cups of unsweetened coffee a day have lower rates of fibrosis, cirrhosis and liver cancer. The protective effect is seen with both caffeinated and decaffeinated coffee.
The key word is unsweetened. Coffee loaded with sugar, syrups, or sweetened creamer doesn't offer the same benefit.
A reasonable weekly target:
If you're starting from zero, don't aim for all of this in week 1. Start with daily walks, build from there.
The well-established target is 7–10% of your body weight. For someone at 80 kg, that's 6–8 kg. For someone at 65 kg, it's 4.5–6.5 kg.
You don't have to reach it all at once. Losing even 3–5% starts improving liver inflammation. The full fat reversal happens at 7–10%. Losing it slowly (0.5–1 kg per week) is more sustainable than crash dieting, which can actually worsen liver stress.
Yes — more than most people realise. A single night of under 6 hours of sleep reduces insulin sensitivity by 20–30%, which directly increases fat storage in the liver. Chronic short sleep is independently linked to fatty liver — even in people whose diet and exercise are fine.
If you snore loudly or your partner notices pauses in breathing, get assessed for sleep apnoea. Untreated sleep apnoea is a major, fixable driver of fatty liver.
